We are a small boutique Approved Home Care Provider under My Aged Care.  We are a Home Care Package managing provider who deliver in-home support for clients, with a focus on delivering personalised excellent client service.  We treat our clients like our own families, and we want them to think of us as their own.

Our service area includes Bayside, Kingston, Frankston and Mornington peninsula areas in metropolitan Melbourne.

We're a boutique Approved Aged Home Care provider in the southern and southeastern suburbs of Melbourne.  We are growing and are now looking to supplement our existing case managers with another case manager.  The role involves: managing prospective clients completing assessments of prospective/new clients and creating care plans and HCP budgets working with our scheduler to schedule care plan services, and manage changes ongoing completing re-assessments co-ordinating services including Allied health, home maintenance, social support regular checkins with clients for welfare checks working with PCAs and other support staff and contractors servicing clients client monitoring and reporting monthly service invoices and statements The role will be performed from home, using your own computer and the online services and software we provider.  You will need a laptop and a smart phone.
