Our Client is currently seeking a passionate Case Manager for Kings Park Location.

They are currently seeking an outstanding and passionate Case Manager who would like to be part of an impressive organisation that lifts the standard of care provided in the community.

Responsibilities:

        1. Person centred assessment and care planning

  • In consultation with clients, carers and families facilitate care assessment, care planning and review
  • Develop and monitor individual plans and budgets with regular reviews/case conferencing
  • Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
  • Apply clinical governance framework to clients requiring clinical care
  • Development and maintenance of all documentation in client management systems
  • Work within legislated NDIS and CDC frameworks
  • Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety
  • Contribute to quality processes such as management of client feedback
  • Actively participate in professional development activities (internally and externally) to maintain contemporary knowledge and skills as well as acquire new knowledge and skills

       2. Client and stakeholder engagement

  • Make relevant referrals, broker services and deal with suppliers as per care plan
  • Develop and maintain relationships with community organisations to ensure a holistic case management approach
  • Develop and maintain relationships with brokerage agencies to ensure quality services are delivered by external
  • Ongoing communication with clients, carers/families to ensure they are aware of rights and responsibilities to make informed

       3. Business Development

  • Ongoing relationship management with relevant referral sources
  • Develop links in the community to source new referrals
  • Retention of existing consumers
  • Participate in Clinical Governance meetings and Continuous Improvement
  • Participate in objectives and performance review to reach KPI’s set in relation to new and existing consumers.

Essential requirements:

  • Knowledge of local and regional networks and service providers
  • Screening the right candidate for quality recruitment.
  • Oversee client questions . 
  • Ability to work with individuals, families and carers from a strength based approach, using high level interpersonal, verbal and written communications skills, together with negotiation liaison and conflict resolution skills.
  • Understanding and experience of current case management practice and principles such as assessment, developing and reviewing care plans, making referrals.
  • Ability to develop quality individual plans that are consisted with principles of person centred planning within NDIS and CDC frameworks
  • Computer literacy skills and experience working with client management software systems.
  • Current Police Check
  • Current Drivers Licence and your own car
  • Current First Aid
  • Current Working with Children's Check for paid employment
